This is a case report on treating 2 children who had uncontrolled bleeding related to spinal fusion surgery in spite of treatment with aprotinin and FFP. Bleeding was controlled with two doses of rFVIIa (90 μg/kg)for each patient. It was also observed that coagulation parameters normalized after giving rFVIIa. The author concludes that this treatment seems to be effective in surgeries associated with dilutional coagulopathy, but that it remains investigational and that randomized studies in such cases are warranted.
